Why does one joint keep getting sore? A worn joint or past injury can cause that repeated ache.

The joint may ache after walking, reaching or standing. It may also feel stiff after a long rest.

Write down the movement that brings on the ache. Add how long the ache remains and what it stops you from doing.

## Daily use often brings the soreness back

A worn knee may hurt on steps or late in a walk. A shoulder may ache when the arm reaches overhead or behind the back.

Standing up may catch in a sore hip. A joint joins bones, while a tendon ties muscle to bone.

Tell the doctor or nurse exactly where the soreness sits. That detail helps separate joint trouble from a sore tendon.

## Gentle movement can help a stiff joint

Short walks, easy motion and light strength work may ease stiffness. Pause the exact activity behind the flare for a while.

Cold may reduce swelling; warmth may ease stiffness. Stop and seek medical advice if movement makes the ache much worse.

Keep a short note of good days and bad days. It can show whether care at home is helping.

Try only one home treatment at a time. Then you'll know which treatment eased the soreness.

Call the doctor's office if the ache doesn't ease after several days. Lasting soreness that blocks normal tasks needs an exam.

## Lasting soreness deserves a proper exam

The visit starts with questions from a doctor, nurse or other trained medical provider. It includes movement checks and a review of any X-ray.

Regenerative care discussed at QC Kinetix can include concentrated PRP, a blood layer with extra clot-forming platelets saved by machine. The findings and past care guide that choice.

Ask the provider what the exam showed and why the proposed care suits it. You can also ask what daily task will show progress.

## Chandler activity can test a sore joint

A long shift or repeated stairs can bring back knee, heel or shoulder soreness. Quick stops at Tumbleweed Park or a longer walk on the Paseo Trail can do the same.

Tell the doctor or nurse which task hurts and when the soreness starts. That is clearer than saying only that the joint feels bad.

1. The DEPA classification was built because platelet and leukocyte counts alone do not describe an injection. Applied retrospectively to 20 published PRP preparations, the dose of injected platelets ranged from 0.21 billion to 5.43 billion - a 25-fold spread. No device recovered more than 90% of the platelets in the blood drawn, and most preparations were contaminated with red blood cells: only three of the devices reached a purity score corresponding to more than 90% platelets relative to red cells and leukocytes.

2. A systematic review of 29 randomized trials of PRP for knee OA that reported a platelet count, concentration or dose found that the 28 treatment arms with statistically significant positive outcomes at 6 months had a mean platelet dose of 5,500 (+/-474) x 10^6, while the three arms without a positive difference averaged 2,302 (+/-437) x 10^6 (P<.01). The same separation held at 12 months: 5,464 versus 2,253 x 10^6 (P<.05).

3. A systematic review and meta-analysis of platelet dosage across musculoskeletal conditions identified a potential dose-response relationship in knee osteoarthritis, with an apparent threshold above 10 billion platelets for favourable clinical outcomes, and the effect more pronounced for function than for pain. For conditions other than knee OA the authors found the literature too unclear to identify an optimal dose.

DOI: 10.1007/s12178-024-09922-x.
4. In the RESTORE trial - the largest placebo-controlled PRP trial in knee osteoarthritis - 288 adults aged 50+ with symptomatic Kellgren-Lawrence grade 2-3 medial knee OA received three weekly intra-articular injections of leukocyte-poor PRP from a commercial system or saline placebo. At 12 months the mean change in knee pain was -2.1 points with PRP versus -1.8 with saline (difference -0.4; 95% CI -0.9 to 0.2; P=.17) against a minimum clinically important difference of 1.8, and the change in medial tibial cartilage volume was -1.4% versus -1.2% (difference -0.2%; 95% CI -1.9% to 1.5%; P=.81). Twenty-nine of 31 prespecified secondary outcomes showed no significant between-group difference. The authors concluded the findings do not support use of PRP for knee OA.

DOI: 10.1001/jama.2021.19415.
5. A meta-analysis of 34 randomized trials (1403 PRP knees, 1426 control knees) found WOMAC favoured PRP over placebo at 12 months (P=.02) and over hyaluronic acid at 6 and 12 months (P<.001), and favoured PRP over steroids on VAS pain and KOOS at 6 months. Critically, the authors reported that the superiority of PRP did NOT reach the minimal clinically important difference for any outcome, and graded the quality of evidence as low.

6. The devices used to spin PRP at the point of care are cleared by FDA as clinical centrifuges, product code JQC, a Class I device under 21 CFR 862.2050 - for example the Biomet GPS Platelet Separation Kit (K030555, cleared 2003) and the Harvest SmartPrep2 / SmartPrep Platelet Concentration System (K103340, cleared 2010). That clearance covers the equipment that separates blood. It is not an FDA approval of platelet-rich plasma as a treatment for osteoarthritis, tendinopathy or any other orthopedic condition, and copy must never blur the two.
